Transitional Enhanced Care Coordination (TrECC) is a family-driven process for helping youth with significant mental and behavioral health challenges, who may be undergoing psychiatric hospitalization or an episode of treatment in a residential setting.
Connected Families NH is seeking individuals to support these youth and their families to successfully transition from one setting to the next, ensuring that the youth is in the least restrictive environment and has access to the appropriate supports in their targeted community. The TrECC Coordinator will maintain a caseload of 8-10 youth and families.
The successful candidate will possess a minimum of a Bachelor’s degree and at least two (2) years experience, preferably in health care, social services, behavioral health, human services or education. Inviduals with lived experience will be considered as well. Strong computer and documentation skills are required. Candidates must have the ability to work independently, yet have the skills to be organized, reliable and creative when working in teams. This position involves regional travel around Sullivan County and lower Grafton County in addition to occasional state-wide travel. Offices in Claremont, NH.
