Maplewood Nursing Home is seeking a Full Time Social Worker

Under the direction of the Social Services Director, responsible for providing ongoing social service case management for the social, spiritual, psychological, emotional, behavioral, medical, and financial needs of residents. Also serves as family liaison.

40 Hours a week, Monday to Friday

Starting Pay of $27.89

  • Functions as resident advocate and liaison with other residents within the facility, with families or representatives, with staff within the facility, clergy, Department of Elderly and Adult Services, Division of Health and Human Services, Veterans Administration, Social Security Administration, Ombudsman’s Office, Office of Public Guardian, hospitals, funeral homes, Hospice, social service agencies, and clothing suppliers.
  • Counsels applicants and representatives, before and after admission, regarding advanced directives, guardianships, end of life issues, nursing home expenses, etc.
  • Assists residents with financial management and budgeting.
  • Documents all significant social service contracts with residents in their medical record and according to Federal, State, and facility guidelines. Those progress notes should include response and adjustment to institutional environment, problems, needs, family contacts, and a plan of care.
  • Participates in the interdisciplinary care planning process according to the federal, state, and facility guidelines for MDS, RAPS, and care planning, being mindful of the designated areas in which social workers are especially responsible (psychosocial, mood, behavior).
  • Assists in placement of residents in other facilities, offering counseling to residents and their representatives.
  • Assists with bereavement counseling for staff and families.
  • Participates in the interdisciplinary discharge planning process, formulates a discharge plan, arranges community support for residents who are discharged home and provides follow-up consultation.
  • Participates in continuing education in-service and workshops within and out of the facility in order to remain knowledgeable about current legislation, trends, and resources as they relate to duties performed.
  • Consults regularly with the Social Services Director regarding resident issues, problems, and changes in status or condition.

SKILLS/EXPERIENCE/TRAINING REQUIRED:
Duties require knowledge of social work and hospice care equivalent to completion of a Bachelor’s degree and one to three years of related experience, or equivalent combination of education and experience.
Experience in casework and geriatric social work desirable.
Thorough knowledge of social and health resources available in the community.
Familiarity with computerized long-term care documentation.
