Connected Families NH is seeking a Care Coordinator for our FAST (Families and Systems Together) Forward New Hampshire Wraparound practice. This position will serve youth and families, who are experiencing significant behavioral and mental health challenges, within their homes and communities. The youths worked with range from 6 -21 in age. The Care Coordinator will become certified in an evidence-based, family-centered practice. Care Coordinators facilitate a planning process focused on monitoring family outcomes and working with the youth, their family, and community partners to support their success at home, in school, and in their community. This position will carry a caseload of 8-10 cases.
The ideal candidate will possess a minimum of a Bachelor’s degree and at least two (2) years of experience, preferably in health care, social services, behavioral health, human services, or education. Individuals with lived experienced will be considered, as well. Experience in group facilitation is preferred. Strong computer and documentation skills are required. Candidates must have the ability to work independently, yet have the skills to be organized, reliable, and creative when working with teams. Applicants must possess a valid driver’s license and vehicle insurance.
The position involves regional travel around Sullivan County and lower Grafton County in addition to occasional state-wide travel. Offices in Claremont, NH.
We also have a position involving travel around Cheshire County with offices in Keene, NH.
