Substance Misuse Prevention Coordinator
The Cheshire County Commissioner's Office is hiring a Full Time Substance Misuse Prevention Coordinator.
Under the direct supervision of the Grants Manager, applicant will be responsible for coordinating efforts around substance misuse in specific settings. The work will involve assessment, planning, capacity building, implementation and evaluation.
Minimum Qualifications: Candidates with a Bachelor’s degree and those working towards certification as a prevention specialist preferred. Certified Prevention Specialist status must be obtained with in one year of hire. Five years’ experience preferred in: community organizing, community development, community relations, substance misuse prevention and organizational development.

Keene NH is seeking a Clinical Case Manager.
Under the direct supervision of the County Administrator, applicant will be responsible for case management of participants of the Drug Court, working with the Drug Court Team, legal, treatment and recovery community. Position is 32/hrs week.
Functions include but not limited to:• Completes GAIN and ORAS assessment on
participants• Develops comprehensive treatment plans and directs clients to
resources.• Conducts weekly monitoring meetings with clients and maintains
continual assessment of progress, including random drug screenings.•

Maintains New Hampshire Physical Therapy license and is responsible to assure continuing education requirements meet board regulations.
Staff RN/LPN - Various ShiftsUnder the direction of the Nurse Manager or RN Supervisor, responsible for performing clinical and administrative duties on assigned floor.
Supports the Nurse Manager or RN Supervisor in maintaining standards of resident care and maintaining policies and protocols to meet OBRA regulations for long term care.
Duties require knowledge of nursing equivalent to completion of an applicable Nursing Program.
Must be licensed by the State of New Hampshire Board of Nursing.
Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
