Overview

Connected Families NH is a county-run care management entity dedicated to helping children, youth, and families in Cheshire County and across New Hampshire access the mental health supports and community resources they need to thrive. While part of Cheshire County government, Connected Families also operates as a specialized agency focused on family-driven, community-based care coordination.

What Connected Families NH Does

Connected Families NH works with families experiencing mental health and behavioral challenges from birth through age 21. Their goal is to connect you with the right supports and services in your community while respecting your family’s culture, comfort, and schedule.

  • Free of charge for families who participate

Who We Serve

Connected Families NH supports children and youth from birth through age 21, including:

Supports families with children ages birth to 5 who are experiencing emotional or behavioral challenges. Early intervention can make a lasting difference for young children and their families. 

Designed for children and youth ages 5 to 21, FAST Forward offers team-based long-term support (typically 12–18 months) to help families build stability and success at home, school, and in the community. 

Helps youth under age 21 as they transition into or out of residential or psychiatric care, ensuring plans and supports are in place for a successful transition back into their community. 

Family-Focused & Community-Based Care

Connected Families NH uses a community-centered care coordination model, meaning services and resources are rooted in the communities where families live. Care coordinators work with families and their natural supports—such as teachers, relatives, and counselors—to create tailored plans that help children reach their goals. 

The approach is family-led, culturally respectful, and designed to reduce stress while helping families access what they need most.
