Overview
We are seeking a dedicated Senior Case Manager to join our team. The ideal candidate will have a strong background in working with chronic and persistent mental health issues, co-occurring disorders and substance use disorders. This position is responsible for diverting criminal justice involved individuals diagnosed with a substance use disorder, co-occurring disorders and/or severe and persistent mental health disorder. Provides clients with the tools they need through treatment, rehabilitative programming, reinforcement, and monitoring in order for them to lead a productive, law-abiding life style.
Essential Functions
- Interviews and assesses all clients to determine appropriateness for Behavioral Health Court programs, ie. Mental Health Court or Alternative Sentencing
- Develops comprehensive individualized service plans and directs clients to resources
- Maintains frequent communications with all provider agencies regarding client progress
- Maintains ongoing communications with House of Corrections, Probation and Parole offices
- Reports to courts on clients’ level and/or violation of compliance
- Conducts weekly monitoring meetings with clients and maintains continual assessment of progress
- Develops and maintains individual records including urinalysis results on each client for review by courts, attorneys, etc.
- Makes decisions regarding the discharge or completion of a client from the mandated program
- Attends monthly Behavioral Health Court meetings and all meetings mandated by Mental Health Court State Coordinator
- Assists coordinator with updating program needs, ie. brochure, handbook, policy and procedures manual
- Provides weekly supervision to clinical case manager and other staff
- Runs appropriate t4reatment groups on an as needed basis
- Skills, Experience, Training Required:
- Knowledge and experience with individuals living with chronic and persistent mental illness, co-occurring disorders and substance use disorders. Must have equivalent to completion of Master’s degree and three to five years of related work experience in these fields or equivalent combination of education and experience. MLADAC and/or LICSW required or working toward licensure.
- Demonstrated knowledge of therapeutic interventions and treatments specific to the client population
- Ability to communicate effectively both verbally and in writing; to establish positive public relations and to interact effectively with a wide variety of people
- Ability to manage a large and varied caseload
- Ability to work on a multi-disciplinary team
- Ability to work under pressure
- Ability to understand and appreciate the dynamics of working with persons with substance use disorder problems, persons in crisis and persons seeking to enhance personal growth and functioning toward more effective living
