Connected Families NH is seeking a Regional Manager for the northern region of our service area (Sullivan and Grafton counties). This full time, benefitted position is located in Claremont, NH and responsible for the oversight of the enhanced care and wraparound practices within the Care Management Entity. Additionally, this person will supervise the regional staff with respect to practice and caseload management, staff training and case documentation; they will also oversee referral pathway for families and manage the day-to-day operations of the northern region.
Connected Families NH, which is under the governance of Cheshire County, is a family-centered service dedicated to providing a care coordination services that are unique to each family. We work with families who are experiencing difficulties due to behavioral and emotional health challenges at home, in school and/or their community. We believe that children, youth, and families deserve services that address their unique needs and incorporates support from family members and their community.
Candidates should have a Bachelor’s degree and a minimum of 5 years of experience working in a collaborative environment; strong supervisory and interpersonal skills and the ability to balance diverse responsibilities.
