Maplewood Nursing Home

Licensed Nursing Assistant - Full Time/Various Shifts

Under the supervision of the LPN Charge Nurse or Staff Nurse, provides basic resident care and assists in the maintenance of the unit environment.

Follows facility policies and procedures and the standards of the Nurse Practice Act of New Hampshire.

Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.



 Staff RN/LPN - Various Shifts

Under the direction of the Nurse Manager or RN Supervisor, responsible for performing clinical and administrative duties on assigned floor.

Supports the Nurse Manager or RN Supervisor in maintaining standards of resident care and maintaining policies and protocols to meet OBRA regulations for long term care.

Duties require knowledge of nursing equivalent to completion of an applicable Nursing Program.

Must be licensed by the State of New Hampshire Board of Nursing.
Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
Must attend mandatory in-services and retain registration with the State of New Hampshire.
 


MNA

GENERAL SUMMARY:

Under the direction of the charge nurse, assists with routine prescribed medication administration and documentation in accordance with established nursing standards, and the policies, procedures, and practices of this facility.  Administers PRN medications upon the instruction of the delegating licensed nurse. 

ESSENTIAL JOB FUNCTIONS:

·        Receive report at the beginning of shift with nurses. 
·
       
Prepare, administer and chart oral, topical, suppository, nasal, ocular, and otc medications, as prescribed by the physician, and as directed by the delegating licensed nurse through shift assignment and Medication Administration Record (MAR).
·
       
Document administration on the MAR.
·
       
Administer PRN medications as prescribed by the physician, only as directed by the delegating licensed nurse. Delegating licensed nurse must be available to monitor the effects on the resident.  Documents same on MAR. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

·        Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.
·
       
Must have successfully completed a state approved Medication Nursing Assistant Program.

Gives prompt and efficient personal care and assistance to residents under the direction of the Administrator or On Call ALF Nurse.

Will be responsible for:

·        Answering resident call lights, and various other duties.

·        Charts required information and signs/dates entries.

·        Give and receives report from the off going shift.  Reports to ALF Administrator or On-Call Nurse

Candidates should be available for shifts on days, nights and weekends.  On call personnel train for eventual part time/ full time employment as positions open. Requires operating complex communications system and the ability to communicate professionally and effectively with the public.  Excellent multi-tasking, typing, and listening skills are required.  Must be able to demonstrate tact and calmness in stressful situations. High School graduate with associate degree or equivalent experience with some police and/or public safety/emergency dispatching preferred. Candidates must pass a battery of tests to be considered.  Successful candidate must pass be able to pass a thorough background check and polygraph examination.

Cultural and Linguistic Competency (CLC) Coordinator

To assist in the planning process of a System of Care

 

Part-time, 20 hr/wk, temporary, grant funded position until 9/29/15

Reports to the System of Care Project Manager

 

The CLC coordinator will assist with the infusion of cultural and linguistic competence within a seamless System of Care for children with serious emotional disturbances. The CLC coordinator is a team leader with a key decision-making role. He/she will be a resource and provide guidance to the expansion team and will serve on the system of care steering committee to ensure culturally and linguistically competent practices in all aspects of the system of care.

 

Knowledge, Skills and Abilities Required:

• Knowledge of cultural and linguistic competence principles and concepts

• Knowledge of organizational change principles and concepts

• Knowledge of systems of care and wraparound philosophy and practice, community based services and supports with an orientation to family-driven, youth guided and culturally relevant systems and services

• Knowledge of the National CLAS Standards

• Bi/multilingual applicants encouraged to apply

 

Prior Experience Required:

• 3 years’ experience with community-based behavioral health systems

• Experience in promoting cultural and linguistic competence or related tasks such as diversity trainer or multicultural coordinator

• Bachelor's degree in human services, social work, sociology, education, psychology, anthropology or other relative field

GENERAL SUMMARY:

Under the general supervision of the Maintenance Supervisor, responsible for maintaining and repairing buildings, grounds, and equipment for the County, ensuring that buildings are in good repair and free of hazards.

ESSENTIAL JOB FUNCTIONS:

·        Performs building maintenance, repair, and renovations, including carpentry, plumbing, mechanical, electrical, and painting work.

·        Maintains exterior grounds, including mowing, raking leaves, weeding, and planting flowers and shrubs.

·        Installs, repairs, replaces, and renovates plumbing and electrical fixtures and broken equipment; performs paint and repair work.

·        Performs routine and unscheduled maintenance on boiler, heating systems, pumps, generators, and other County equipment.

·       Ensures safety precautions are followed at all times during the performance of work duties.

 

 SKILLS/EXPERIENCE/TRAINING REQUIRED:

Duties require knowledge of mechanical, plumbing, electrical, maintenance work, and operations equivalent to completion of a high school diploma along with advanced courses from a two year college or technical school and one to three years of related experience, or equivalent combination of education and experience. 

Must possess a valid State of New Hampshire driver’s license

Working knowledge of the methods, work standards, practices, tools, and materials used in the mechanical, plumbing, and electrical trades
