Licensed Nursing Assistant - Full Time/Various Shifts

Under the supervision of the LPN Charge Nurse or Staff Nurse, provides basic resident care and assists in the maintenance of the unit environment.

Follows facility policies and procedures and the standards of the Nurse Practice Act of New Hampshire.

Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.



 Staff RN/LPN - Various Shifts

Under the direction of the Nurse Manager or RN Supervisor, responsible for performing clinical and administrative duties on assigned floor.

Supports the Nurse Manager or RN Supervisor in maintaining standards of resident care and maintaining policies and protocols to meet OBRA regulations for long term care.

Duties require knowledge of nursing equivalent to completion of an applicable Nursing Program.

Must be licensed by the State of New Hampshire Board of Nursing.
Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
Must attend mandatory in-services and retain registration with the State of New Hampshire.
 


MNA

GENERAL SUMMARY:

Under the direction of the charge nurse, assists with routine prescribed medication administration and documentation in accordance with established nursing standards, and the policies, procedures, and practices of this facility.  Administers PRN medications upon the instruction of the delegating licensed nurse. 

ESSENTIAL JOB FUNCTIONS:

·        Receive report at the beginning of shift with nurses. 
·
       
Prepare, administer and chart oral, topical, suppository, nasal, ocular, and otc medications, as prescribed by the physician, and as directed by the delegating licensed nurse through shift assignment and Medication Administration Record (MAR).
·
       
Document administration on the MAR.
·
       
Administer PRN medications as prescribed by the physician, only as directed by the delegating licensed nurse. Delegating licensed nurse must be available to monitor the effects on the resident.  Documents same on MAR. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

·        Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.
·
       
Must have successfully completed a state approved Medication Nursing Assistant Program.

Ensures adaptive equipment is available for residents.
·       
Serves meal trays, assists with tray preparation, removes trays after meals.
·       
Completes Appetite Records.
·       
Clears dining room after meals.
·       
Reports appetites less than 25% to nursing staff.
·       
Returns food carts to kitchen area.

·        Sets objectives and standards for the shift.

Evaluates residents’ status and unit activities at the beginning of each shift and assures an appropriate level of nursing staff are available to provide safe, high quality care.

Participates in and assures appropriate assessment of residents.

Coordinates and communicates information about care with residents and families.

GENERAL SUMMARY:

Under the direction of the Director of Occupational Therapy, responsible for evaluating, treating, and monitoring residents’ abilities to safely consume nourishment and effectively communicate in their daily lives.  All responsibilities are in accordance with governmental, state, and facility practices and standards.

ESSENTIAL JOB FUNCTIONS:* 

·        Organizes and conducts resident’s speech-language program to facilitate rehabilitation or habilitation of a resident. 

·        Consults with other members of the team in order to coordinate with other therapeutic/rehabilitative activities. 

·        Selects rehabilitative and therapeutic treatment based on evaluations; determines a treatment program appropriate for the resident.   

·        Works closely with Dietary and Occupational Therapy staff on program recommendations. 

 

Duties require knowledge of speech language pathology equivalent to completion of a Master’s degree and three to five years of related experience, or equivalent combination of education and experience.  New Hampshire Allied Health Professional’s license to practice as a SLP required.  Must attend mandatory in-services and retain license with the State of New Hampshire.

ESSENTIAL JOB FUNCTIONS:

·        Organizes and facilitates group programs, including bowling, movies, bingo, cooking, arts and crafts, holiday and special party groups, mini bus rides, shopping, luncheons, and musical and social functions in and outside the facility.
·       
Adapts activities to Resident’s current abilities.
·       
Implements individual activities for the residents, including letter writing, supportive social visits, board games, outdoor activities, individual projects, outside wheeling or walking, and one-on-one sensory and tactile awareness to meet the Activity Department 1 to 1 visit policy. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

·        Duties require knowledge of activities for geriatric nursing home residents. Equivalent to completion of a high school diploma plus additional courses from a two year college or technical school and one to three years of related experience, or equivalent combination of education and experience.  Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.

Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
·       
Must attend mandatory in-services and retain LNA registration with the State of New Hampshire.

Must possess a valid driver’s license.

Under the supervision of the Housekeeping Supervisor, responsible for performing a wide range of cleaning duties in all areas of the Nursing Home, using specialized housekeeping equipment and chemicals in order to maintain a clean, safe, and aseptic environment for residents, staff, and the public..

ESSENTIAL JOB FUNCTIONS:

·        Follows cleaning schedule as outlined for Housekeeping Department to include discharged/isolations cleaning procedures.

·        Cleans all assigned building areas throughout the Nursing Home in a clean, neat, and sanitary manner and in accordance with established housekeeping procedures.

·        Operates various types of cleaning equipment, both mechanical and electrical, in order to remove soils, stains, and other debris from assigned areas.

·        Cleans all resident rooms and corridors on a daily basis; disinfects when necessary; polishes where required.
