Maplewood Nursing Home

Licensed Nursing Assistant - Full Time/Various Shifts

Under the supervision of the LPN Charge Nurse or Staff Nurse, provides basic resident care and assists in the maintenance of the unit environment.

Follows facility policies and procedures and the standards of the Nurse Practice Act of New Hampshire.

Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.



 Staff RN/LPN - Various Shifts

Under the direction of the Nurse Manager or RN Supervisor, responsible for performing clinical and administrative duties on assigned floor.

Supports the Nurse Manager or RN Supervisor in maintaining standards of resident care and maintaining policies and protocols to meet OBRA regulations for long term care.

Duties require knowledge of nursing equivalent to completion of an applicable Nursing Program.

Must be licensed by the State of New Hampshire Board of Nursing.
Must be thoroughly oriented to all facility policies and agree to comply with all facility policies.
Must attend mandatory in-services and retain registration with the State of New Hampshire.
 


Ward Aide

·       Under the supervision of the Staff Nurse, responsible for assisting residents and staff with non-direct resident care activities. 

ESSENTIAL JOB FUNCTIONS:

·        Prepares dining room for meals.
·
 
      Ensures adaptive equipment is available for residents.
·
       
Serves meal trays, assists with tray preparation, removes trays after meals.
·
       
Completes Appetite Records.
·
       
Clears dining room after meals.
·
       
Reports appetites less than 25% to nursing staff.
·
       
Returns food carts to kitchen area.

MNA

GENERAL SUMMARY:

Under the direction of the charge nurse, assists with routine prescribed medication administration and documentation in accordance with established nursing standards, and the policies, procedures, and practices of this facility.  Administers PRN medications upon the instruction of the delegating licensed nurse. 

ESSENTIAL JOB FUNCTIONS:

·        Receive report at the beginning of shift with nurses. 
·
       
Prepare, administer and chart oral, topical, suppository, nasal, ocular, and otc medications, as prescribed by the physician, and as directed by the delegating licensed nurse through shift assignment and Medication Administration Record (MAR).
·
       
Document administration on the MAR.
·
       
Administer PRN medications as prescribed by the physician, only as directed by the delegating licensed nurse. Delegating licensed nurse must be available to monitor the effects on the resident.  Documents same on MAR. 

SKILLS/EXPERIENCE/TRAINING REQUIRED:

·        Duties require knowledge of nursing assistance equivalent to completion of a high school diploma plus additional courses and up to one year of related experience, or equivalent combination of education and experience.

Must be registered as a Licensed Nurse Assistant by the State of New Hampshire.

Must have successfully completed a state approved Medication Nursing Assistant Program.

Cheshire County has an opening for a PCA in the Assisted Living Facility at Maplewood Nursing Home in Westmoreland.

Gives prompt and efficient personal care and assistance to residents under the direction of the Administrator or On Call ALF Nurse.

Will be responsible for:

·        Answering resident call lights, and various other duties.

·        Charts required information and signs/dates entries.

·        Give and receives report from the off going shift.  Reports to ALF Administrator or On-Call Nurse

Assists in maintaining good public relations with staff, management, residents and the public

Under the supervision of the Laundry Supervisor, responsible for the washing and folding of laundry, ensuring that clean linens and personal clothing are delivered.

ESSENTIAL JOB FUNCTIONS:

·        Collects soiled linens and assists with sorting and stain removal.

·        Removes linens from dryers and sorts by resident name and/or location.

·        Hangs, folds, counts, and stacks linens according to department or unit.

·        Places linens and Attends on carts and distributes to department according to their predetermined daily needs.

·        Inspects linens for wear or sets aside for repair.

·        Must be 18 years or older.

Under the supervision of the Housekeeping Supervisor, responsible for performing a wide range of cleaning duties in all areas of the Nursing Home, using specialized housekeeping equipment and chemicals in order to maintain a clean, safe, and aseptic environment for residents, staff, and the public..

ESSENTIAL JOB FUNCTIONS:

·        Follows cleaning schedule as outlined for Housekeeping Department to include discharged/isolations cleaning procedures.

·        Cleans all assigned building areas throughout the Nursing Home in a clean, neat, and sanitary manner and in accordance with established housekeeping procedures.

·        Operates various types of cleaning equipment, both mechanical and electrical, in order to remove soils, stains, and other debris from assigned areas.

·        Cleans all resident rooms and corridors on a daily basis; disinfects when necessary; polishes where required.

·        Obtains food and supplies as instructed.

·        Performs cleaning duties in the kitchen and dining room; gathers dirty dishes from the resident floors; scrapes dishes, loads dishwashing machine; and puts away clean dishes.  Ensures a sanitized and clean environment.

·        Works on the tray line; cuts up breakfast tickets.

·        Assists in serving meals and loading food.

·       Assists in setting up dining room, including setting up juices, butters, desserts, and breakfast and lunch specials for the following day.

Candidates should be available for shifts on days, nights and weekends.  On call personnel train for eventual part time/ full time employment as positions open. Requires operating complex communications system and the ability to communicate professionally and effectively with the public.  Excellent multi-tasking, typing, and listening skills are required.  Must be able to demonstrate tact and calmness in stressful situations. High School graduate with associate degree or equivalent experience with some police and/or public safety/emergency dispatching preferred. for the Children’s Behavioral Health Collaborative (NHCBHC) 

Part-time, 20 hr/wk, temporary, grant funded position until 9/29/15

Reports to the System of Care Project Manager

 

The Cultural and Linguistic Competency Coordinator will assist with the infusion of cultural and linguistic competence within a seamless System of Care for children with serious emotional disturbances. The CLC Coordinator is a team leader with a key decision-making role. He/she will work with leadership, management, staff, families, youth, contractors and all other system partners in ensuring culturally and linguistically competent practices in all aspects of the system of care.

 

Knowledge, Skills and Abilities Required:

•Knowledge of cultural and linguistic competence principles and concepts
•Knowledge of organizational change principles and concepts
•Knowledge and expertise in systems of care and wraparound philosophy and practice, community based services and supports with an orientation to family-driven, youth guided and culturally relevant systems and services 

Prior Experience Required:

Three years’ experience with community-based behavioral health focused on children, adolescent and family services. Experience in promoting cultural and linguistic competence or related tasks such as diversity trainer or multicultural coordinator, etc.

Cheshire County
located in Keene NH seeks a

Project Manager for the Children’s Behavioral Health Collaborative
to develop a seamless System of Care for children with serious emotional disturbances. 

Full time, temporary, grant funded positon until 9/29/15. 

The Project Manager, will work closely with the collaborative and various community stakeholders to coordinate the planning of a system of care in the Monadnock region that results in an integrated, comprehensive children’s behavioral health system plan that includes an asset map of current services, a gap management plan for the expansion of needed services, and a workforce plan. 

Responsibilities include but not limited to:

  • Managing the objectives and activities outlined for the Project
  • Working collaboratively with facilitators, consultants and partners to reach the goals of the project
  • Conducting forums to educate stakeholder groups about the Project’s goals and outcomes;
  • Enhancing the membership of the Collaborative

Qualifications: This position requires a minimum of a Bachelor’s degree from a recognized college or university and a minimum of five years professional work experience preferably in health care, social services, behavioral health care, social work, human services, public policy or education, with demonstrated project management experience.  Work experience may be substituted for a portion of the educational requirement.  This position requires strong leadership and management skills plus the ability to work independently.
